The caring and professional staff at Hilltop of Greenville Memory Care strives to understand the changing challenges that memory loss presents to its community members, and to use that knowledge to craft care plans that are as unique as each Hilltop of Greenville resident.
Hilltop of Greenville Executive Director Marisa Smith said the process begins from the first day.
“When a new resident joins our community, we conduct a thorough baseline assessment that looks at their cognitive, physical and emotional health,” she said. “This initial assessment includes a series of customized questions designed to evaluate memory function, daily living skills and overall well-being. These questions and observations are tailored to the individual’s unique needs and abilities, allowing us to create a care plan that truly reflects their current state and anticipates future needs.”
The success of the care plans, according to Smith, is measured “through resident satisfaction, family feedback and clinical outcomes.”
Smith also pointed out that each care plan is continuously monitored.
“One of the key aspects of our approach is the flexibility to adjust care plans as needed,” she said. “For example, if a resident shows signs of increased memory loss or a decline in physical health, their care plan is immediately reviewed and modified to address these changes. Adjustments could include increasing the frequency of certain activities, introducing new therapies or modifying the level of assistance provided with daily tasks.”
Staff members don’t manage care plans in a vacuum. Smith said community members’ families are also involved in care planning.
“At Hilltop, we are a family, and everyone is involved every single day,” she said. “These residents are our family. We also value the input from families, as their observations and experiences are crucial in evaluating the effectiveness of our programs.”
